Pre-Participation Forms

ALL CURRENT AND INCOMING ST. MARY'S COLLEGE ATHLETES
Below you will find updated information on health record submission required for athletics for the 2026-27 academic year. The following is required for all student-athletes:
  • Sports Medicine Packet (completed by student-athlete on Sportsware)*
  • Sports Physical Form (form completed and signed by qualified Healthcare Provider and uploaded to Sportsware)
  • Insurance Holder Signature Page  (completed by student-athlete on Sportsware)*
  • SMHAT-1  (completed by student-athlete on Sportsware)
  • Sickle Cell Trait Status - 1st year and transfers only (lab results uploaded to Sportsware)
  • Baseline Neurocognitive Concussion Exam (completed with team on-campus)
*If the student-athlete is under 18 years old, this section must be completed by a parent or guardian

It is a requirement that all St. Mary's College student-athletes have personal health insurance. If you do not have a personal health insurance plan, please notify your team coach or Head Athletic Trainer DJ Hayes.

All forms must be returned by AUGUST 1, 2026 through Sportsware.

ST. MARY'S COLLEGE ATHLETES will use SportsWare Online to create a profile (first-years/transfers only), complete online forms (all student-athletes), upload their physical form (all student-athletes) and sickle cell trait results (required for all incoming first-year/transfer student-athletes).
 
The NCAA mandates that all student-athletes have knowledge of their sickle cell trait status. Athletes have the following options:
  • Show proof of previous sickle cell test
    • Most newborns are tested for the sickle cell trait and this information should be easily accessible from your primary care doctor or the State Newborn Screening Center at your State Health Department.
  • Consent to a sickle cell solubility blood test to check for the sickle cell trait
    • This simple blood test can be performed at any laboratory. Contact your primary care physician for a script if you are unable to retrieve previous sickle cell test results. Indicate this is a NCAA requirement for participation in collegiate athletics.

SPORTSWARE ONLINE
FIRST-YEARS, TRANSFERS, AND CLUB SPORTS STUDENT-ATHLETES
Incoming first-years, transfers, club sport student-athletes, and/or any student that has NEVER tried out for a varsity or club sports team before should follow: New Student-Athlete SportsWare Online directions.

RETURNING STUDENT-ATHLETES

ADDITIONAL NCAA CLEARANCE DOCUMENTS
If you are currently taking ADD/ADHD medication, anabolic agents, hormone and metabolic modulators, peptide hormones, or growth factors related substances, or any other NCAA banned substance, you must have your doctor sign the document below along with providing medical documentation as listed on the form to the Sports Medicine Staff by August 1.
